I've been taking on the challenge of getting S rank on Nightmare Mode without unlockables, and I finally managed to do so a few days ago.

This is the damage chart of the nightmare difficulty.
The number in the colored cells indicates rounding value of 'enemy HP/weapon damage', which means you have to fire shots of that number to kill the enemy.
* green cells - I'll mainly use these weapons against the enemy.
* red cells - unnecessary or impossible attacks, overkill
Weapons
- Although It has high damage per shot, I won't use the VP70 and pick up its ammo. The VP70 cannot be switched to single shot fire, which means you'll inevitably miss 1-2 shots and it'll decrease your accuracy.
- I'll use P345 only when I fight with the Doctor.
- The hunting rifle has extremely high damage. It also can be used to kill Suicider with a single shot. You can find 30 rounds in the single player campaign, and I'll try to pick up all of them.
- When you use the weapon like the switchblade, the Glock, Its melee bash can stun some enemies just one time.
- When you use the weapon like the shotgun, the M16, the hunting rifle, Its melee bash can stun all enemies (except Faster) for an infinite amount of times until you run out of stamina.
Enemies
- Enemies can kill each other. (very important)
